Job Summary:
To provide supplemental patient support for provider treatment plan and assist family, caregiver, and other significant caregivers with navigating patient psychosocial and long term care needs.

Job Duties:
  1. Along with the counselor, works with families after receiving a diagnosis by all providers in the Glenn Biggs Institute clinic through individual, couples and family therapy to improve mental health and coping skills in facing a new diagnosis.
  2. Develop innovative support and community engaging programming for caregivers and patients to promote the mission of the Glenn Biggs Institute and encourage involvement.
  3. Identify patient and caregiver needs. Evaluate and identify current patient status to include: caregiver support, legal issues such as Power of Attorney, current community resources that are in use, insurance needs of the patient, and educational resources such as caregiver workshops, legal and insurance.
  4. Provides recommendations to family and patient on issues such as: community support resources, patient and caregiver educational needs, long term care and residential care, hospice, in-home care, home health services, legal resources, therapy and counseling, psychiatric needs, long-term planning, financial resources, insurance, and social work liaison/Advocacy.
  5. Counsels families and patients on community resource development such as: residential facility outreach and contact networking, hospital provider/ outpatient clinic resource contact, therapy, counseling, psychiatric, crisis intervention resource contacts/resources, Insurance, Medicare/Medicaid, V.A. benefits, long-term care contacts, clinic outreach, public presentations and education.

Education:
Masters degree in social work is required.
Licenses as a Social Worker in Texas (LMSW, LSW)

Experience:
Three (3) years of professional social work experience is required.
Prefer Bilingual English/Spanish
