Advanced Practice Provider (Physician Extender and Patient Navigator)

The Glenn Biggs Institute for Alzheimer’s & Neurodegenerative Diseases in the Joe R. and Teresa Lozano Long School of Medicine at UT Health San Antonio seeks an Advanced Practice Provider (APP) with training or experience in Behavioral Neurology, Geriatrics, Dementia, Vascular Neurology, and/or Movement Disorders.

The faculty candidate will join a highly collaborative, multidisciplinary team that includes neurologists, neuropsychologists, nurses, social workers, geriatricians, geriatric psychiatrists, therapists, counselors, and care managers. Experience caring for individuals with Alzheimer’s disease, related dementias, or other neurological conditions is especially welcome.

Position Overview

This position plays a critical role in supporting patient referrals and care needs acting as a clinician extender for those seen by Dr. Sudha Seshadri, Director and Physician, within the Glenn Biggs Alzheimer’s and Dementia Clinic. The APP will also coordinate donor-related referrals in collaboration with the Office of Institutional Advancement. The role includes managing all aspects of the director’s patient referral workflow and care, ensuring a seamless experience from referral initiation through follow-up. Clinical excellence, strong judgment, and discretion are essential.

Knowledge, Skills, and Abilities

  • Excellent clinical skills and knowledge; prior experience in neurology, psychiatry or geriatrics with some knowledge of Alzheimer’s care is preferred.

  • Solid understanding of healthcare systems, including insurance processes, medical terminology, and regulatory requirements

  • Proven capacity to manage multiple priorities in a fast-paced environment

  • Proficiency with electronic health records (EHRs) and Microsoft Office applications.

Essential Job Duties

Director Referral Management

  • Oversee all aspects of the patient referral process for the physician, ensuring a seamless and timely experience from initiation through follow-up.

  • Triage incoming referrals, schedule appointments, and coordinate communications among referring physicians, specialists, and patients.

Clinical Duties

  • Perform physical examinations, order laboratory and imaging studies, make preliminary diagnoses, and provide treatments.

  • Assists the physician in diagnostic and therapeutic management by performing a variety of medical or procedural tasks.

  • Instruct and counsel patients on diagnosis, treatment plans, including new anti-amyloid treatments, enrollment in clinical trials and health related topics.

  • Obtain comprehensive medical histories, conduct examinations, and present findings and recommendations in a clear, clinically useful manner to support the physician in decision making.

Clinical Liaison

  • Collaborate with physicians, nurses, and allied health professionals to ensure referral pathways align with clinical protocols and standards of care.

  • Provide guidance and support to clinical staff to help facilitate timely access to services.

  • Serve as a consistent point of contact for patients navigating the referral process and advocate for timely access.

  • Maintain accurate, up-to-date records of referrals, communication logs, and outcomes.

  • Generate reports and analyze referral volumes and trends to identify opportunities for process improvement.

Performs all other duties as assigned.

Education requirements

  • Licensed NP or PA with at least 2-3 years clinical experience.

  • This is a role in which mentoring by APP peers and clinicians will be abundant.

Benefits Overview

Disability- Employees can enroll in the Disability Insurance which provides income if a non-work related illness or injury prevents you from working.

 

FSAs- Employees can enroll in flexible spending accounts (FSAs) to set aside money from earnings before taxes for qualifying dependent day care expenses or out-of-pocket health care expenses.

 

Retirement- Employees are eligible for either the Teacher Retirement System (TRS) or the Optional Retirement Plan (ORP). TRS is a defined benefit retirement plan which UT Health matches employee contributions. ORP is for eligible faculty staff employees. Voluntary retirement programs are also available to invest before- or after-tax dollars with the choice of five quality retirement plan providers.

 

Time Off- A generous leave program offers multiple paid leave options:

 

Front-loaded Paid Time Off: 128 to 208 hours (16 to 26 days) of Paid Time Off based on years of service, given at the start of each fiscal year. PTO may be prorated in year one based on date of hire.

 

Extended Illness Bank: 8 hours (1 day) accrued per month which can be used for illness or injury after one day of Paid Time Off is taken.

 

Paid Family Leave: Up to 240 hours (6 weeks) to care for a spouse, child, or parent after 6 months of consecutive employment.

 

Holidays: 12 set paid holidays each fiscal year.
