Job Summary:

This position is responsible for providing UT Health San Antonio Primary Care patients with education, support, and navigation services that are tailored to patient’s identified needs and health profile. The candidate will be responsible for ensuring that patients receive safe, quality, and valuable, patient-centered support by collaborating with members of the Population Health Team.  


Must be able to work a flexible schedule and travel throughout San Antonio to assist with event execution and complete training as needed.

Job Duties:

  1. Counsels patients with appropriate and relevant educational materials.
  2. Assists patients in connecting to appropriate health care information and services, addressing barriers to receipt of follow-up services and treatment.
  3. Provides patient reminders to upcoming appointments and next steps of treatment.
  4. Serves as liaison between patients, health/social services, and the community to enable access and improve the quality and competence of service delivery.
  5. Builds and maintains positive working relationships with individuals and workgroups through open communication and collaboration; works with others to accomplish goals and objectives.
  6. Demonstrated knowledge of health and social services systems and familiarity with community resources and services.
  7. Performs all other duties as assigned.



High School Diploma or GED is required.



One (1) year of community-based or clinical setting or related experience is required.



Bilingual in Spanish is preferred.

Proficiency in Microsoft Office Suite.


Additional Licenses and Certifications:

Successful completion of a Community Health Worker formal training program such as from a college or other education institution is preferred.
